How much does TRT cost in Canada?
There is no single price for testosterone treatment in Canada. The bill is made of separate charges, and services differ in which ones they charge for, which they fold into another fee, and which they do not mention at all. The table below puts what each provider publishes for medication beside an estimate of what that comes to per month and per year, with the assumption in the same cell. The seven categories of published charge follow, each with its billing interval.
Two kinds of number are on this page and each says which it is. A published charge is exactly what the provider displays, with the page and the date. An editorial estimate is this site's arithmetic from those published charges, shown with its assumptions and its exclusions. Where a fee is not published, the estimate is a subtotal plus unconfirmed fees, not a total. Products and quantities differ between providers and are not equivalent, so the table does not say which is cheaper.
- Who is included
- Every provider in the dataset, alphabetically, including those that publish nothing for a given category.
- Captured
- 12 September 2026, dataset v0.2, province, scope, price and pharmacy fields re-checked 23 September 2026. Figures change; check the provider’s page before relying on one.
- Three kinds of figure
- Published by provider, with the page and the date checked. Confirmed directly with provider, with the date, the method and the responding role. Editorial estimate, a calculation by this site with its assumptions shown beside it. Every figure says which it is.
- Missing data
- Written as not confirmed. A published figure is never altered, and an estimate is never shown as a quoted price.
- Conflicts
- Where a provider’s own pages disagree, both statements are kept and the difference is set out under the field.

How much does TRT cost per month and per year in Canada?
It depends on which number you mean. The medication alone is the smallest part: provincial formularies price a 10 mL vial of generic testosterone cypionate at $34.88, before the pharmacy's markup and dispensing fee. A full care budget adds the visits, the blood work, any service or renewal fees and the dispensing fee on every fill. Across the 5 services that publish enough to estimate, this site's yearly subtotals run from $636 to $2,860, or about $53 to $238 a month when a year is spread over twelve. All amounts are in Canadian dollars. The parts of the bill and the estimates by provider follow, each with its date and its assumptions.
What each part of TRT costs in Canada
Before any one provider, the bill has the same parts everywhere: the visits, the blood tests, the medication, the pharmacy's dispensing fee and, for injections, the supplies. The table gives what each part costs on its own, from provincial formularies, lab price lists and fee schedules, each with its source and date. Medication prices are per vial, packet or capsule, because how long a vial lasts depends on the prescribed dose.
| Item | Cost | Details and sources |
|---|---|---|
| Doctor or nurse practitioner visits, public route | No charge to you | Insured by provincial health insurance (OHIP, MSP and the other provincial plans) when you see a family physician, a nurse practitioner, a walk-in clinic or a specialist on referral. |
| Blood tests ordered by a clinician | Usually no charge to you | Provincial plans insure total testosterone when a clinician orders it for a medical reason, and pay the laboratory directly: $14.48 for a total testosterone test in Ontario and $15.81 in British Columbia. | Pharmacy dispensing fee | About $5 to $13 a prescription | BC PharmaCare says pharmacy fees are usually $5 to $13 and covers up to $11. The Ontario Drug Benefit pays $8.83, or $9.93 to $13.25 at rural pharmacies, plus a markup of 6% or 8% of the drug cost. Paying cash, you pay the pharmacy's own fee. | Injection supplies | About $35 to $63 for 100 syringes; about $9 for a sharps container | Boxes of 100 syringes with needles at online medical suppliers, and a 1 L sharps container from St. John Ambulance. No dates shown on those pages. Some services supply them for a charge or include them; each provider's figure is in the tables below. United Canada, BD 3 mL syringes with 25G needles (no date shown); TherapySupply.ca, syringes with needles (no date shown); St. John Ambulance, sharps container 1 L (no date shown) |
| Online or private service fees | No separate charge up to $170 a month, among the services in this dataset | Membership, programme, renewal and consultation fees vary more than any other part of the bill. Each provider's published charges, with their billing interval, are in the tables below. |
| Part | Public route (family physician, nurse practitioner, specialist) | Online or private service |
|---|---|---|
| Visits and prescribing | Insured by your provincial plan. | Depends on the service: some charge no separate fee, others charge a consultation, renewal or monthly fee. |
| Blood tests | Insured when a clinician orders them. | The service sends a requisition to a local lab, and the test is insured or not under the same provincial rules. Some services also sell private panels. |
| Medication | You pay, unless a public or private drug plan covers it. | The same: coverage follows the prescription, not the service. Some services dispense from their own pharmacy, others send the prescription to a pharmacy you choose. |
| Dispensing fee | Charged by the pharmacy, per prescription. | Charged by the pharmacy that fills it, separately or included in the medication price depending on the service. |
Whether your province's drug plan pays for the medication, and who each plan covers, is on the drug plan coverage page. Every cypionate product listed in Canada, with its vial price and supply record, is on the testosterone cypionate page.
What do I pay before my first prescription?
Before treatment starts there is usually an intake or assessment, an order for blood work, the blood work itself and a consultation to review it. Services differ in which of these they charge for. The table shows what each provider publishes for those steps and for the dispensing fee at the first fill, next to this site's estimate for a first year and for an ongoing year, so the start-up cost is not confused with the running cost. "Not confirmed" means the service does not publish the charge; it is not a zero.

Estimated overall TRT costs
What a year of treatment is estimated to come to with each provider, on the same basis, for an ongoing year rather than a first year. Each figure is this site's arithmetic from the charges that provider publishes, and each is a subtotal plus the fees that provider does not publish. Nothing here says which provider is cheaper for any particular person: the products and the quantities differ, coverage differs, and what is missing from one estimate is not missing from another.
Ongoing year, not the first year. Estimates calculated 30 September 2026 from published charges and re-verified before publication. The monthly figure spreads the yearly estimate over twelve months so that two providers can be compared on the same basis; it is not a bill anyone sends. Every figure is a subtotal plus unconfirmed fees, so it is a floor rather than a total, and products and quantities differ between providers and are not equivalent.
| Provider | Estimated overall monthly cost | Estimated overall yearly cost | What is included, and what is not |
|---|---|---|---|
| UPGUYS | $53 to $103 Known charges only. | $636 to $1,236 Plus unconfirmed fees. Editorial estimate. | Estimate, supplies and cancellation terms not stated Medication from the company estimate, filled three times a year, plus the $11.99 dispensing fee per fill. Not included: injection supplies, laboratory testing. Calculation and assumptionsFrom: Medication $200 to $400 per four-month supply (cost article); dispensing fee $11.99 per prescription (FAQ); follow-up and standard shipping no separately stated charge (service page). Arithmetic: Medication 3 x $200 to $400 = $600 to $1,200; dispensing 3 x $11.99 = $35.97; follow-up $0; shipping $0; sum $635.97 to $1,235.97, rounded to $636 to $1,236. Assumes: Three fills a year, one per four-month supply, as the cost article describes. The article does not say whether its $200 to $400 figure includes the dispensing fee, so the fee is added; if it is already included, the estimate counts it twice. Laboratory tests are assumed to be provincially insured. Excludes: Injection supplies are not included; no price is stated. Any lab charge for a test your province does not insure is not included. |
| Jack Health | $73 to $157 Known charges only. | $872 to $1,880 Plus unconfirmed fees. Editorial estimate. | Partial estimate Medication and quarterly renewals, both as published on the testosterone page. Not included: lab requisition, laboratory testing, dispensing fee, injection supplies, membership. Calculation and assumptionsFrom: Medication $36 to $120 per month; renewal $110 every three months; shipping no separately stated charge (testosterone page). Arithmetic: Medication 12 x $36 to $120 = $432 to $1,440; renewals 4 x $110 = $440; shipping $0; sum $872 to $1,880. Assumes: A renewal is charged every three months. The page describes the renewal as "$110/ every 3 months depending on your treatment"; see the note on the provider record. Excludes: Requisition, dispensing and supplies are not published and are not included. Biomarker testing is described as included in the assessment and renewals. A $40 visit fee applies if the prescription goes to an outside pharmacy. |
| Essential Clinic | $109 to $122 Known charges only. | $1,312 to $1,460 Plus unconfirmed fees. Editorial estimate. | Partial estimate Annual membership and injection medication. Most other charges are not published. Not included: laboratory testing, dispensing fee, injection supplies. Calculation and assumptionsFrom: Annual consult fee $49, covering the initial assessment, lab requisitions, clinician oversight and follow-ups; shipping free (FAQ); injections as above. Arithmetic: Medication 12 x $105.23 to $117.61 = $1,263 to $1,411; plus $49; shipping $0; sum $1,312 to $1,460. Assumes: Injections, not topical. The consult fee is charged once a year. Excludes: Laboratory testing, dispensing and injection supplies are not published and are not included. The FAQ says follow-up blood tests are covered by provincial health plans in most cases. |
| Science & Humans | $127 to $157 Known charges only. | $1,520 to $1,880 Plus unconfirmed fees. Editorial estimate. | Partial estimate Quarterly programme fee and one vial a month, with the outside-pharmacy dispensing fee. Not included: laboratory testing, shipping, injection supplies, interim consultations. Calculation and assumptionsFrom: Medication $60 to $90 per vial; programme fee $200 every three months (programme page); dispensing $25 when an outside pharmacy is used (FAQ). Arithmetic: Medication 12 x $60 to $90 = $720 to $1,080; programme fees 4 x $200 = $800; sum $1,520 to $1,880. Assumes: One vial per month. Four programme fees a year. Medication filled at the provider's pharmacy, so the $25 outside-pharmacy fee does not apply. Excludes: Interim consultations at $100 each are excluded because their number is not published. Laboratory testing, shipping and supplies are not published and are not included. |
| Enhanced Men's Clinic | $192 to $238 Known charges only. | $2,300 to $2,860 Plus unconfirmed fees. Editorial estimate. | Partial, and the published prices conflict Subscription, medication and blood testing, from the pricing page. The home page describes a different monthly bundle and the two have not been reconciled. Not included: dispensing fee, shipping. Calculation and assumptionsFrom: Subscription $170 per month; medication $50 to $100 per ten weeks; blood testing $0 to $300 per year; other supplies $50 per year if needed (all pricing page). Arithmetic: 12 x $170 = $2,040; medication 12 x $21.67 to $43.33 = $260 to $520; blood testing $0 to $300; sum $2,300 to $2,860. Optional supplies not counted. Assumes: The subscription is charged every month for a year. Follow-up is treated as covered by the subscription because it is not separately stated. Optional supplies are not counted, the same way optional supplies are left out for every provider. Excludes: Dispensing and shipping are not published and are not included. Optional supplies, $50 per year if needed, are not included. The home page states $176 per month combined, which is a separate provider statement. |

Is TRT covered by provincial health plans or private insurance?
- Clinician visits: insured by your provincial health plan when you see a family physician, nurse practitioner or specialist. Online and private services are not covered by public plans.
- Laboratory tests: insured when a clinician orders them for a medical reason. Some tests are uninsured and charged by the lab, and tests you order yourself are paid privately.
- Medication: covered by your province's drug plan only if the plan lists the product, you meet its criteria and you are someone the plan covers, then after its deductible or co-payment. Private insurance may cover it instead.
In most provinces a family physician or nurse practitioner visit is publicly insured, and the blood test that confirms low testosterone is publicly covered when a clinician orders it for a medical reason. What varies is the medication. British Columbia requires Special Authority for injectable testosterone. Ontario lists the injections, gels and oral capsules under Limited Use, which requires confirmed morning low testosterone with a documented cause. Most other provinces list the injections as regular benefits and put gels behind special authorization. The status in all ten provinces, with who each plan covers and the date it was checked, is on the drug plan coverage page.
Online and in-person private services are not covered by public plans. Some private fees are eligible under workplace or personal insurance, which is a question for the insurer rather than the provider.

Does a free consultation mean TRT is free?
No. A service that charges nothing for the consultation still charges for the medication, and the pharmacy that fills it charges a dispensing fee, separately or inside the medication price. Lab tests your province does not insure, renewals and supplies may also be charged. Read what a published offer includes, and treat anything it does not mention as not confirmed rather than free. The first-prescription table shows what each service publishes.
Why do online TRT prices differ?
Four things explain most of the difference: the billing interval (a month, three months, a four-month supply or a year), the product and quantity assumed, whether a membership or programme fee is charged on top of the medication, and how much of the bill a service publishes at all. A lower subtotal with more unpublished charges is not a cheaper service.
- "No consultation fee" is not the same as no cost. Ask what the pharmacy charges to dispense, what the laboratory charges, and what a renewal costs.
- A lab requisition is the order for blood work. It is not the blood work, and it is not a statement that the blood work is publicly covered.
- A price for a seven-week or ten-week supply is not a monthly price. The billing interval matters as much as the number, which is why the estimate column converts it and says how.
- A subscription that includes clinical care may exclude medication, and the reverse happens too.
- Promotional first-visit pricing is not the ongoing price, which is why the first year and an ongoing year are estimated separately.
Next: compare the providers field by field, or read what follow-up on treatment involves so the ongoing charges make sense.
Common questions
How much is TRT per month in Canada?
Spread over a year, and counting every fee each service publishes, this site's estimates work out to these amounts a month, lowest first: UPGUYS about $53 to $103; Jack Health about $73 to $157; Essential Clinic about $109 to $122; Science & Humans about $127 to $157; Enhanced Men's Clinic about $192 to $238. These are subtotals: lab tests and any fees a service does not publish are extra. A headline monthly fee on its own is not the full cost.
Is TRT covered by provincial drug plans?
Often in part, with conditions, and only for people the plan covers. Alberta, Manitoba, New Brunswick, Newfoundland and Labrador, Nova Scotia and Saskatchewan list injectable testosterone as a regular benefit. The Ontario Drug Benefit lists it under Limited Use and BC PharmaCare under Special Authority, both of which require a documented cause. Gels are the least covered form. What you pay then depends on the plan's deductible or co-payment. All ten provinces are set out on the drug plan coverage page.
How much does testosterone cost in Canada without insurance?
Provincial formularies price a 10 mL vial of generic testosterone cypionate at $34.88 and a 5 mL vial of generic enanthate at $35.59, each holding 1,000 mg. Generic 1% gel is $2.96 a 5 g packet and oral undecanoate $0.56 a capsule. The pharmacy adds its markup and a dispensing fee, usually $5 to $13 a prescription. A TeleTest survey reported cash prices of $32.99 to $52.79 for a cypionate vial at four pharmacy chains, fee included. How long a vial or pack lasts depends on the prescribed dose.
How much is 3 months of testosterone in Canada?
It depends on the form and the prescribed dose. At the dose in the Canadian testosterone cypionate monograph, 200 mg every two weeks, three months uses about 1,300 mg, a little more than one 10 mL vial. Provincial formularies price that vial at $34.88 before the pharmacy's markup and dispensing fee, and a June 2026 survey found cash prices of $32.99 to $52.79 a vial. Blood tests, service fees and supplies are extra. Your own dose is set by your prescriber.
How much does a vial of testosterone cypionate cost in Canada?
Provincial formularies in Ontario, Alberta and Saskatchewan price a 10 mL vial (1,000 mg) at $34.88, and Newfoundland and Labrador lists $38.02 with its markup included. The pharmacy adds its markup and dispensing fee. On 28 September 2026 one cypionate product was marketed in Canada; the testosterone cypionate page has the product list, prices, drug plan status and supply record.
What costs are hidden in TRT pricing?
The charges most often left out of a headline price are the medication itself, dispensing fees, renewal or programme fees every few months, lab tests, injection supplies and shipping. The tables on this page show, for every provider, which of these it publishes, which it includes in another fee, and which it does not mention.
